Building Access Request Form

Please allow up to 7 business days to process all requests.

This form is used for EOU staff, faculty, students, and partners to request building keys, door codes, and proximity access. If you are a partner to EOU (e.g. OSU) and do not have an ID number, please fill out this form prior to putting in a request. This form includes the reporting of any lost or stolen keys.
